Crony capitalism allowed those companies to use government to ensure that they have no competition, then they raise the price that insurance companies and government will pay before they are willing to not pay, and then they set that price right on that area. The exceedingly high costs of certain drugs like insulin are caused by completely different mechanisms. That is exactly how the open market should work. They will both study the market to determine the most we are willing to pay and then either charge just a bit more or a bit less depending on if it's the government or a business and what their goal is (to encourage or discourage).
